I am a licensed Marriage & Family Therapist in the state of Florida. The insurance providers and EAPs that I accept are listed below. If your insurance is not listed, I would be considered an Out-of-Network provider. For these companies, a billing statement can be used to submit to your insurance company. Out-of-network reimbursement may be possible depending on your insurance plan; a percentage of the session fee may be covered or may go towards your deductible. We can contact your insurance company and determine what your out-of-network benefits are and if you want to apply them to our work together.
A sliding scale may be arranged.
